NHS Gloucestershire Clinical Commissioning Group (CCG) seeks expressions of interest from suitably qualified organisations that have the necessary capability and capacity to provide an Out of Hospital Service which offers short term practical solutions to facilitate a return home from hospital.
As well as supporting the individual to adjust and regain confidence as they recover, the service will also offer longer term support through relevant community networks and resources of the provider or by contact with VCS groups and networks.
The service should work alongside health and social care professionals in primary, community and secondary care to: Prevent avoidable hospital admission; Enable safe and timely discharge for individuals who are medically stable and fit for discharge in: o Emergency Department; o Inpatient Departments.
The service should offer short term support for recovery from ill health or hospital admission, with longer term support using the organisation's experience and knowledge of community networks and support.
NHS Gloucestershire CCG intends to award a new contract for an initial period of 3-years with options to extend for 2 further periods, each of 1 year duration.
Organisations interested in providing this service should request a Pre-Qualification Questionnaire (PQQ) by contacting Paul Wilkinson, Procurement Specialist, NHS Gloucestershire CCG at paul.wilkinson2@nhs.net no later than 5pm Monday 16 May 2016.
Completed PQQ's must be returned no later than 5pm, Friday 20 May 2016.
