This is a notice for social and specific services in accordance with Directive 2014/24/EU Article 74 The Royal Borough of Kingston-upon-Thames has awarded a contract to a collaborative working model for the provision of a unified service to provide support to individuals to promote resilience, health and well-being, providing access to information and advice and supporting carers, jointly with Kingston Clinical Commissioning Group.
The contract will provide the following services: ?

Supporting hospital admission avoidance and support on discharge The Council aims to achieve the following outcomes: People are socially connected and resilient and are able to contribute to their community.
Sub-outcomes: a) People have as much control over their own lives as they want b) People can spend time doing things they enjoy or value c) People are able to develop their social network sand have regular social contact with individuals with similar needs or interests and can foster and maintain these relationships, promoting integration d) People have encouragement and support when required (including peer support) e) People are able to integrate and access universal services i.e. primary care and sport and leisure f) People have access to information on universal services and information on targeted advice and advocacy services available to them g) People are motivated to contribute to their local community with opportunities available for them to use their skills, knowledge and experience to help others to live healthily and independently h) People are supported to volunteer and contribute to their community by doing their bit i) Socially excluded or disadvantaged people are supported to be empowered as equal and active members of their local community in achieving the above outcomes
