The primary objective of the service is to facilitate discharge from hospital and prevent admission to hospital or residential care by providing a Home Care and Support Service over a period of up to 72 hours until a permanent Care Provider can provide the care, in addition to this , any service provided should achieve the outcomes identified by the process of a Care Act Assessment and Support Planning and agreed with the people accessing the service and their unpaid carers. \r The Organisation commissioned to deliver against the specification will enter into a Block Contract Agreement with the Council. \r A Home Care and Support Fast Response Service will be required to be adaptable and responsive to ensure the type of services, required by the person accessing the service are available, and keep pace with new ways of working and new technologies.
The types of service to be delivered are outlined in Appendix 1 for guidance (this list is not complete or exhaustive).\r The workforce providing the Home Care and Support Fast Response Service will be required to operate flexibly and have the ability and skills to organise their resources in the best way possible to achieve the outcomes specified in a personalised and safe way.
The Home Care and Support Fast Response Service will be innovative and adopt a method of learning and improving through practice.
