Norfolk County Council and the 5 Clinical Commissioning Groups in Norfolk are seeking to deliver a service to meet the needs of unpaid carers in Norfolk that will: a.
Maintain their own health and wellbeing. b.
Remain safe and well through building resilience. c.
Allow them to continue in their caring role with confidence. d.
Access clearly defined routes in to social care when needs are complex. e.
Use established approaches for managing and avoiding breakdown.
There is an expectation that the provider will use their skills, knowledge, expertise and networks and work with carers to shape the way services are delivered.
The service will: - deliver targeted support and assisted self-service to provide resources, tools and information available to carers in their communities, - support access to more formal routes in to care, - enable carers to respond in a crisis and plan for emergencies.
