Community Ophthalmology Service
Community Ophthalmology Service Introduction Shropshire Clinical Commissioning Group invites tenders for the above Service (for both Shropshire CCG and Telford and Wrekin CCG).
The Commissioner is seeking a Lead Provider able to deliver a Service which can meet the health needs of the local populations of Shropshire CCG and Telford and Wrekin CCG through a Community based service delivery model and use of innovative service redesign.
The Service The Commissioner wishes to receive responses to the Invitation to Tender (ITT) from suitably qualified and experienced Ophthalmology providers with the necessary capacity and capability (or a demonstrable ability to provide the necessary capacity and capability) to provide a Community Ophthalmology Service.
The Service shall deliver a 'one stop shop' service, with diagnostic and clinical assessments being undertaken at the same appointment, to ensure timely access to specialist clinical expertise in a community setting.
The Service Provider shall be expected to work with the Commissioner to develop the processes necessary for further ophthalmic conditions to be treated/managed in a Community Based Ophthalmology care setting throughout the duration of the contract.
The Service shall:- • Provide timely access to specialist clinical expertise in a community setting. • Deliver a 'one-stop shop' service with diagnostic and clinical assessments being undertaken at the same appointment. • Improve access and reduce waiting times for ophthalmic services, with a flexible appointment system to meet the needs of Service Users and carers. • Provide advice, information and support to primary care practitioners. • Support and enable Service Users to manage their own conditions by providing a supportive and proactive approach to self-management and to signpost Service Users to services that can support them to self-manage. • Ensure Certification of Vision impairment is completed and provision of support aids where required. • Provide a Service that is multi-disciplinary and patient focused. • Provide access to high quality, safe clinical care that gives timely advice, appropriate support, assessment, diagnostics and treatment for patients according to their individual need. • Provide care closer to home, where appropriate. • Commission a high quality service that integrates with other providers. • Generate a reduction in unnecessary out-patient appointments. • Create a Service that uses innovative methods of service delivery.
