The service aims to ensure that school-aged children and young people (CYP) aged 5-19 years are supported to live healthy safe lives and have appropriate access to healthcare to minimise health-related impediments to achieving their full potential.
The service will work with all state-funded schools in Tower Hamlets - 70 primary, 19 secondary schools, and 3 special schools with a total population of over 40,000 CYP.
The School Health and Wellbeing Service is based on the national specification and guidance which are underpinned by extensive evidence on the impact of public health services in improving outcomes for school age children and young people.
The service delivers the Healthy Child Programme for 5-19 year olds and is provided at four levels: • Community: includes developing effective links with Primary Care, health needs assessments, school health profiles; and contributing to development of health related school policies. • Universal: prevention and early intervention services as set out in the Healthy Child Programme 5-19, including vision and hearing screening, the National Child Measurement Programme (NCMP), health checks and health promotion. • Universal plus: supporting CYP with long term conditions and complex health needs, emotional and mental health needs, and problems with school attendance where these are associated with health issues for the CYP; completion of Individual Health and Care Plans (IHCPs) for children with health needs. • Universal partnership plus: Working with vulnerable CYP including those where there are safeguarding concerns and special educational needs.
