PROJECT OVERVIEW Currently the Long Term Conditions and Cancer Programme of the Camden Clinical Commissioning Group (CCG) is undertaking a large scale change initiative to improve the detection, diagnosis and management of disease states such as hypertension (HT) within primary care.
The proposed project is a pilot initiative in 6 GP practices to support this over-arching framework by, • promoting individuals above the age of 40 years, who are not diagnosed with HT and have not been screened for the condition to visit their GP to have their blood pressure measured and recorded (targeted detection) • supporting the 6 GP practices to promote patients who are visiting the site to use the Surgery Pod (a device which automatically measures blood pressure (BP)) to screen for HT (opportunistic detection) The CCG is working with the Design Council on this pilot initiative.
In order to support the pilot GP sites the CCG is procuring two services with distinct but related aims, Service 1: To create materials which the practices can use to promote both opportunistic and targeted detection Service 2: Support practices with the delivery of opportunistic detection of HT by (i) Providing front line staff (non-clinical) education about hypertension (ii) Delivery of BP checks on site This service specification relates to the aims of Service 2.
SCOPE OF WORK 1.
Creating curriculum for education sessions in line with the GP practice’s local protocol, and incorporating the materials produced by the design agency 2.
Coordinating delivery of the education sessions based on the timing and availability of the GP practices 3.
Coordinating the delivery and length of the on-site BP checks with each of the GP practices 4.
Promoting the use of the Surgery Pods in addition to checking BP manually 5.
Promoting general awareness on HT and detection of HT within the waiting area of the GP practices 6.
Coordinating timelines with the design agency and the 6 GP practices availability Further details available in documentation attached.
