Aims Ensure Individuals have the opportunity to exercise choice and control, develop their own solutions, and support to shape their lives with the services they require.

To enable Individuals to achieve a level of independence appropriate to their aspirations, choices, abilities and needs.

To provide appropriate practical assistance to enable Individuals to live independently and actively participate in the local community.

To provide a planned approach, working with Individuals, their families, and other relevant agencies, to enable move on where needs arise.

To ensure that people living in the house are safeguarded and are able to live lives free from abuse, discrimination and harassment.

General Overview Supported living for 5 (five) adults with complex Learning Disabilities.

These Individuals may have additional needs in relation to their mental health, physical disabilities or autism.

The service provider will work with Care Managers, the Commissioning Team and Sovereign Network Group (The Registered Social Landlord) to support the individuals and their families in the transition and ongoing support within this service.
