Yorkshire and Humber Transforming Care Partnerships Enhanced Community Living Framework - Mayman Lane
Yorkshire and Humber Clinical Commissioning Groups, Local Authorities and NHS Specialised Commissioners are seeking to develop a region-wide framework consisting of 10-15 providers to work in partnership with local people, to provide Enhanced Community Living Services for people aged over 16 years who have high levels of complex needs and can be challenging to support in their community.
In particular services are designed to support people who are currently in hospital or in their community who may: • display behaviours that challenge • pose risks to themselves or others • have a history of offending • have experienced multiple placement breakdowns in the community • have spent a long time in a hospital inpatient setting Providers will deliver proactive, personalised models of care, embedding effective risk management and positive risk taking in the least restrictive way.
Providers will demonstrate the required forward-thinking, skills, training and support systems and collaborate with external partners to maintain and/or establish resilience within services and across the regional framework.
Providers are expected to work with Local Authority housing services and registered social landlords to secure a range of affordable settled accommodation options to suit individual needs.
This will range from small, homely single person accommodation; to accommodation as part of a scheme with adapted fixtures and fittings, sensory/therapeutic indoor and outdoor space and shared facilities; to appropriately proportioned and specialist-designed individual properties.
A new complex in Batley West Yorkshire is under construction and will house 6 clients.
NHS Barnsley, Calderdale, Greater Huddersfield, North Kirklees CCGs and Barnsley, Calderdale and Kirklees Councils have undertaken a joint call off competition to enable service delivery to the client group.
