Bath and North East Somerset Clinical Commissioning Group ("the CCG") is considering their options for the continued commissioning of a Community Fracture and Soft Tissue Injury Service for all adults aged 18 or over who are registered with a GP in Bath and North East Somerset.
The aim is to provide a comprehensive service for adults with a suspected fracture involving assessment, intervention, evaluation, advice and support.
This will be in line with national and local best practice guidance, and local population requirements.
This community based Fracture and Soft Tissue Injury Service sees patients with a range of injuries who have been referred to the service by their GP or A+E.
The service offers a complete follow up service and includes expert medical advice, physiotherapy, plaster removal and renewal and X-rays where needed.
The Community Fracture and Soft Tissue Injury Service will consist of: -Primary care referrals into the service -Provision of professionally trained staff e.g.
GPwSI, plaster technicians -Provision of regular clinics within the geographical area to offer increased access -Specialist assessment and treatment e.g. x-ray -Appropriate interventions and onward referral where necessary -Information on and signposting to any relevant support services -Treatment to prevent hospital admissions -Restore maximal functionality -Drive innovation through service improvement via utilisation of available national and local data/guidance, and, the collection and analysis of regular and appropriate service user feedback. -Work with the CCG on appropriate KPIs and strive to meet (and exceed where possible) these agreed performance targets -Ensuring appropriate data collection methods to submit quarterly reports to the CCG as per the information and quality schedules -Form/maintain good positive links to other associated services/providers to allow the provision of multi-agency health and social care -Work alongside referrers to ensure timely and appropriate referrals; this may include training and support -CQC registration -Have appropriate governance and policies -Working closely with Primary Care Networks Approximate annual caseload: 1747 Approximate current number of monthly appointments: 145 Current contract value: £135k per annum.
