The brief

Request for Information for Primary Care Medical Support into Intermediate Care at Sir John Mason House.

The provider will provide medical input into the service in line with Primary Care General Medical Services (GMS) standard contract.

To include: essential services for people who have health conditions from which recovery is generally expected, terminally ill or suffering from chronic disease.

The provider will work closely with registered facility staff, community therapy and nursing staff and other health and social care professionals; to ensure patients receive holistic, integrated care focused on returning them back to independence.

The provider will provide core primary medical care services to inpatients of the facility (in line with GMS contract requirements).

The service will aim to: • Provide the day to day Primary Care service to residents within the registered facility. • Provide attendance at scheduled Multi-Disciplinary Team (MDT) meetings. • Provide response to the unit both routine and urgent primary care in hours (out of hours care is provided via Out of Hours service). • Contribute to development and implementation of procedures and processes to support holistic delivery of patient care. • Access IT solutions, such as video consultations, and have access (preferably to directly input) to patient information records. • Positively contribute to the planned care of these patients to reduce the need for out of hour's services.
