In recent years, LBC has invested significantly in the provision of care for patients with Severe and enduring Mental Illness (SMI), yet despite this investment there has been little to no improvement in outcomes for service users or their families / carers.
LBC and BLMK ICS want to change this by doing things differently and have commissioned a new project, titled PHM at Place: Improving outcomes for those with severe and enduring mental illness (SMI) to explore how outcomes can be improved through investment in new and innovative capabilities and ways of working.
LBC is seeking a partner to link health and non-health council data to explore whether risk markers exist in council-held datasets that would allow earlier intervention with SMI patients to improve outcomes.
