• To provide support, information and guidance to people who are identified as having risk factors for stroke or who have suffered a first stroke (including Transient Ischemic Attack (TIA) also known as a mini stroke) by establishing and providing an individual lifestyle change programme based on their identified risk factors; • To offer stroke prevention advice, encouragement and support to the service user/stroke survivor and family via personal visits, telephone contacts and written material; • To work predominantly with people who have had a TIA and are identified as high risk by the Stroke Consultant, or Stroke Nurse; • To refer service users/stroke survivors and families to other organisations and professionals for specific support in terms of stroke prevention, where appropriate; • To be aware of potential problems which may arise with the service user/stroke survivor and their families/carers, re-referring if appropriate to relevant services; • To evaluate each patient/family to monitor success of the lifestyle change programme agreed.
