The brief

The Employment Support Service facilitates the return or first entry to training, or employment of people assessed as having faced barriers due to their vulnerability or lack or previous employment experience, skills or formal qualifications.

The service prioritises people with mental health support needs, facilitating improvements in and broadening out of skills and abilities, self-esteem and confidence, focussing on practical employment and training issues.

The service works in partnership with relevant local employers and training agencies to create and enhance employment and training opportunities, promoting the employment and training agenda and contributing to local, regional and national strategy.

Care Journey Coordinators provide a dedicated resource to support people through their discharge journey from hospital into the community by supporting people back home.

Coordinators case manage and coordinate a caseload of people, supporting reducing length of stay in hospital and becoming a key point of contact for service users, their families and staff.
