EK C4G Canterbury and Rural Transitional Beds
This request for Expressions of Interest is published on behalf of NHS Canterbury and Coastal CCG for The Canterbury and Rural Town Team GPs.
Purpose The purpose is to engage with potential providers to assess interest in undertaking a small scale pilot for patients with a short-term acute illness who would otherwise have been admitted to hospital.
The service must be delivered in locations convenient for patients resident in Canterbury City Centre area.
Service The objectives of the service are: - to prevent hospital admission for patients requiring short-term care and support. - to work in partnership with community teams for example, Local Referral Unit, District Nurses, transport providers and GPs. - to offer high quality patient centred care to patients. - to provide service users with good experience of care. - to pro-actively plan for the patients discharge ensuring appropriate support is in place to facilitate timely discharge back to the patient’s own home.
Process The attached Expression Of Interest (EOI) document contains the service description, the engagement process description, and a questionnaire for potential providers to summarise their capabilities to provide the service, and to give their views on service delivery and funding.
We wish to hear from potential providers with their responses to questionnaire.
Based on the outcome of this engagement, we will determine the commissioning approach and the appropriate procurement strategy.
