NHS Wyre Forest Clinical Commissioning Group wish to seek Expressions of Interest (EOI) from Care Home providers to provide 5 Care Home Beds to support reablement for people who require a safe, high quality environment whilst receiving short term interventions which are person centred and multi-agency, in order to: • Improve hospital discharge, transfer from hospital and promote rehabilitation • Reduce avoidable admissions to hospital • Avoid premature/avoidable dependence on long-term care.
