Urgent Care Patient Coordination and Placement Service
Urgent Care Patient Coordination and Placement Service Service to be provided across Northamptonshire Requirements NHS Nene CCG and NHS Corby CCG are seeking expressions of interest from suitably qualified and experienced providers to provide a patient coordination and placement service.
This service will support the flow of patients through the urgent care and hospital system and into the most appropriate pathways for the patient.
This service will actively coordinate and support a defined cohort of patients into existing interim pathways, within clear timeframes while providing a seamless transition and single point of contact for the patient and their families.
To provide: • Support and assistance to families in identifying appropriate care homes (residential and nursing) for the patients who are self-funding or referred for support. • Co-ordination and management of the CHC D2A pathway for assessment working with partner organisations • Co-ordination and management of the Non Weight Bearing Pathway (NWB) on behalf of the CCG • Co-ordination and management of 'spot purchase placements' across the system on behalf of the CCG.
