The brief

The Council is seeking submissions from suitably skilled and experienced organisations to provide Dementia Advisory Service to people who have dementia and their supporters, family and carers.

The annual value of the contract is £63,000 and the initial term is for 3 years with an option to extend for a further 2 years subject to satisfactory performance and continued demand for the service.

The overall aim of the Dementia Advisory Service is to empower people who are affected by and/or suffering from dementia to be able to "live well with dementia" by the following means: 1.1 Service accessibility; local presence and actively seeking out those affected by dementia and that are traditionally found hard to reach.

1.2 Focus on the individual, empowering them to access the information they need, promoting independence, self-help, well-being and true choice and control.

1.3 Provision of quality information and signposting which is tailored around the specific needs of the person's circumstances.

1.4 Collaboration with other health and social care professionals and active development of these partnerships to maximise the outcomes for the person with dementia.
