WCC - Hospital transport and settling service
Warwickshire County Council is seeking tenders from providers who are interested in providing safe and timely transport home for eligible adults (age 18/65 and above), who reside in Warwickshire, that have been discharged from an Accident and Emergency (A&E) department overnight.
The puropose of the service is to prevent overnight admission into hospital for adults who are well enough to go home but lack the transport or require support in settling in when they are back home.
Key requirements of the service are: This service is for the provision of non-registered support - the service excludes the provision of personal care.
The service will be required 12 hours a day (8pm to 8am), 7 days a week and 365 days a year.
A&E discharge may be from South Warwickshire General Hospital, George Eliot Hospital or University Hospitals Coventry and Warwickshire.
The provider will need to attend the relevant A&E department within 60 minutes of the referral being made.
The provider will need to remain with the customer for as long as required, in order to ensure that the customer is settled back into their home.
This may be until the morning time if in a care home or until the patient's paid/unpaid carer is due to visit (if they are in receipt of a community support service).
The provider's staff will need to have recent DBS checks.

These services may be modified and changed in order to meet the needs of the Council or the customers, or as a result of changes to the level of funding available (either up or down).
