The overall aims of the Integrated Community Anticoagulation service will be to: Provide a cost effective, standardised, clinically effective and safe Integrated Community Anticoagulation service; Improve quality and clinical outcomes through standardisation of care across an integrated pathway; Enable patients to have equitable access to a range of medications and treatments in a convenient community or domiciliary setting; Provide comprehensive and ongoing education to support medication adherence and patient self-management.
Further, to the previous Market Engagement exercise completed in August 2017, NHS Croydon Clinical Commissioning Group (CCG) is inviting suitably qualified and experienced Providers of Anticoagulation Services to an Outcomes Framework Engagement Event taking place in Croydon on the evening of Thursday 7th June 2018.
You are invited to CONFIRM YOUR INTEREST and attendance at the Outcomes Framework Engagement Event by reviewing the updated service specification and completing the attached Market Engagement Questionnaire Service.
Deadline for expressions of interest, in the form of a completed Engagement Questionnaires is Monday 4th June 2018, by email to tom_baker@nhs.net Additional information The duration of the contract will likely be for 3 years with the option to extend for a further 2 years at the CCG's discretion.
The CCG intends for the service to commence in 1st April 2019.
The indicative annual contract value for this service is approximately £1,500,000.00 Estimated date of publication of contract notice: 19/07/2018
