Call-Off 1 was awarded under the sole Provider Framework Agreement for D/deaf Services DN676909.\r \r This Call-Off Contract is with Nottinghamshire County Council only and will run for an initial term of 3 years, with an option to extend for up to a maximum of 3 years from the end of the Call-Off Contract initial term. \r \r This Call-Off Contract will deliver a prevention-focused offer which will build connection, improve access to information and skills to support self-care, independence and well-being, and address issues that could escalate and have a negative impact on well-being or independence.\r \r The Provider will do this through a combination of three approaches: \r \r • A 'Community Organising' approach that will connect D/deaf people, their families, carers and wider communities and motivate them to share skills and assets to achieve common goals and build capacity/social capital.\r • Partnership working to support D/deaf people's priority goals, create opportunities, and address inequality in access and outcomes. \r • Delivery of brief interventions, including welfare rights support, to support well-being, prevention and navigation.
