The model is focused on a more proactive management of frail older people and improving their long term outcomes.
The commissioner is NHS Ashford CCG as Lead Commissioner and NHS Canterbury and Coastal CCG, NHS South Kent Coast CCG and NHS Thanet CCG as Associate Commissioners.
