NHS Gloucestershire Clinical Commissioning Group (CCG) seeks expressions of interest from suitably qualified organisations that have the necessary capability and capacity to provide a county-wide peer support and befriending service for people living with or caring for stroke, dementia or other Long Term Conditions.
The service will contribute to improved health and social outcomes for individuals and families by: • Delivering a model of peer support and befriending that: o Incorporates disease prevention and risk reduction through promoting a healthy heart, healthy mind approach o Is strengths based and person-centred o Promotes self-care through enabling and empowering individuals o Is accessible within local communities across Gloucestershire • Works closely with health, social care, voluntary, charitable services and community groups to support healthy living and recovery from ill health • Provides skills to people with lived experience living with stroke, dementia or other Long Term Conditions to deliver peer support and befriending as a Volunteer Befriender The present service is provided by a single provider.
NHS Gloucestershire CCG intends to award a new contract for an initial period of 3 years with option to extend for a further 2 year period.
The successful bidder will receive an annual contract value of £47,400 per annum for the contract period (including the 2 year extension option).
