The brief

We are seeking suitably qualified and experienced providers of Home (Domiciliary) Support are invited to tender for the delivery of additional preventative Home Support capacity in Stockport with a focus on `Wellbeing, Independence, Responsive and Ethical provision'.

This additional opportunity is part of an overall strategy to provide further options to support people to live at home, providing a specific opportunity to test and develop different models of service delivery in home support.

The service is designed to work with others to support people in the community and help prevent crisis situations such as hospital admission and carer breakdown.

It is being led by SMBC Adult Social Care (ASC) as part of the `Stockport Together' neighbourhood plan to provide additional home support capacity across the borough.

It will work in close partnership with the `REaCH' in -house service to identify individuals who can benefit most readily from a re-ablement approach.

The focus on this service will be on `step up'; offering additional support to prevent people needing to be admitted into care homes or hospital where that is possible and appropriate.
