The NHS Health Check programme is aimed at detecting those most at risk of Cardiovascular Disease (CVD) and will be offered to patients aged 40-74 years old.
This programme is provided by GPs in Leicester City who are given a payment for every patient seen and if a patient is placed on a management plan a further payment is given.
A patient who is assessed as not needing to be placed on a management plan, then the patient is recalled to have a further check every 5 years until either they are ineligible through age (or in the intervening time placed on a disease register for a condition).
In the event that a patient is assessed as needing to be placed on a clinical pathway to have their conditioned managed then a one-off payment is given to the GPs, and the patient is no longer invited for a Health Check.
This contract is awarded through Direct Award Process B under the Provider Selection Regime.
