Nottinghamshire County Council is seeking a single Provider to deliver a range of short-term Home Based Care and Support Services across the County of Nottinghamshire (excluding Nottingham City) using a Framework Agreement.
The Framework will replace a similar Framework Agreement which is expiring and will be available for use by the following Commissioners: • Nottinghamshire County Council • NHS Bassetlaw Clinical Commissioning Group • NHS Nottingham and Nottinghamshire Clinical Commissioning Group Bidders MUST have demonstrable experience of delivering services that are similar in scope and size to those described in the specification.
This includes the delivery of Rapid Response Reablement services under contract to a Local Authority or Clinical Commissioning Group (CCG), with a combined annual contract value of at least £1.5m.
The Provider must be registered with the Care Quality Commission (CQC) to provide home care services for the duration of the Framework Agreement and Call-off Contracts.
The Provider must also maintain a permanent office within Nottinghamshire.
In addition to establishing the Framework Agreement the Council will enter into contracts for an initial 2 call-off orders for existing services: 1) Home First Response Service; Community Service (preventing admission to hospital or care home) and Hospital Discharge 2) 24-Hour Response Service Further work may be called off during the life of the Framework Agreement
