Tenders are invited by The Council of the Borough of Kirklees (the "Council") from Service Providers/Contractors with relevant experience and ability to demonstrate sufficient capacity for Kirklees Stroke Information, Support & Targeted Prevention; Six month Reviews following Hospital Discharge; and Communication Support (the "Service").
It is anticipated that is to commence on the 1st October 2019 and will expire on 30th September 2023 (2+2 (4 years).

The total contract value is £136,000 per annum.
