The brief

To support people with mental health issues (including people with Dementia) who have been awarded an indicative budget to consider options for meeting their needs.

The provider will work with the individual accessing support, their Carer(s) (where appropriate) and their care manager to co-productively agree a Support Plan.

The Support Plan will be used to agree the solutions required to meet identified needs.

The Service Provider will then support the individual involved to purchase services and from there to ensure that all the required options are in place.

Where peer brokers offer advice / help, they will be well-placed to observe the choices people make, and can feed this information through to the commissioning process.

This will help identify gaps in support.

This contract is for an initial 3 years with the option of 2 further 1 year extensions.

The annual estimated amount is £52,000
