The overarching aim of the Service is to help to reduce inappropriate admissions to hospital or premature admission to long term residential care, by providing a high-quality service delivered by staff that are appropriately qualified and trained to enable people with Dementia to live independently as long as possible. <br/><br/>The Contract will be for a term of two years with the opportunity to extend for one year and a further one year subject to mutual agreement between the successful Bidder and the Authority.
The maximum term of Contract is consequently four years.<br/><br/>The available budget is £245,759 per annum.
