Fortnightly face to face clinic at Northside House for 1 session (either always a GP, or a monthly GP clinic and a monthly Nurse Practitioner clinic who could alternate to provide 1 session per fortnight).

This is a secure mental health unit, and the contract will run over a 12 month period.

Primary care review/advice for chronic disease management which will include, but not limited to, as below: • Primary care asthma care & reviews from trained practitioners • Primary care COPD care & reviews from trained practitioners • Primary care diabetes care & reviews from trained practitioners - to support in-patient staff.
