The brief

The service is designed to support people to return home as soon as they are medically fit for discharge.

People will be able to return home safely and supported to prevent readmission into hospital services.

Aims and Objectives - Support people to return home safely from hospital and prevent readmission wherever possible. - Provide support to older people (60+) living in the community to prevent admission into hospital.

The aspects of this service are: (1) Support older people to prepare them for leaving hospital (2) Provides a facilitated discharge to adults of all ages from hospital settings for residents of Bath and North East Somerset (B&NES).

This can include an element of practical housing-related support, cleaning, repairs, or minor adaptations to people's homes to help facilitate their discharge from hospital, or to remain safe once discharged from hospital. (3) Low-level support to older people in the community who are at risk of admission into hospital.
