Cwm Taf Morgannwg University Health Board is intending to award a contract for Dietetic Led Diabetes Remission Service.
It is proposed that the Adult Weight Management Service (AWMS) develop a remission service as an extension of their current offer, working with: • Primary care to target recently diagnosed T2DM patients who are ready to undertake an intensive intervention • People who are newly diagnosed following their attendance at the group based structured education intervention Xpert • Level 3 weight management service patient cohort A Total Diet Replacement (TDR) intervention will be procured for use by the service enabling patients to achieve significant weight loss, improve their healthcare outcomes and reduce health inequalities associated with overweight and obesity.
