NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board (BNSSG ICB) and NHS Gloucestershire Integrated Care Board are currently working together in a formal "clustering" arrangement to prepare for their merger into a single new organisation (name to be confirmed) effective from 1st April 2027.
BNSSG ICB (the commissioner) is seeking to secure a suitably qualified provider who has the capability and capacity to deliver a community-based pre-diabetes programme in South Bristol.
The service will provide a preventative intervention for adults aged 18-50 diagnosed with pre-diabetes, aiming to reduce progression to Type 2 diabetes, improve health outcomes, and address health inequalities.
It will operate as a person-centred, community-based model integrated with primary care, complementing existing services such as the National Diabetes Prevention Programme (NDPP).
The service includes: • 1:1 assessment within 6 weeks of diagnosis and a personalised support plan • Education and self-management support to enable sustainable lifestyle change • Follow-up at 6 months to review progress and outcomes • Navigation to local services, acting as a link between primary care, community provision, and voluntary sector support The service will focus on reaching underserved populations and provide the service across community settings, including evenings and weekends.
The contract will be for an initial 3 year term with an optional extension of any period up to a further 2 years.
The new service will commence on the 18th January 2027.
This procurement is being carried out by NHS South, Central & West (SCW) on behalf of the commissioners.
SCW is currently subject to restructuring, and the management of this procurement may therefore move to a successor at some point during the procurement process.
Bidders will be informed of any such change, and any administrative changes because of it.
