Aims and Objectives - Support people to return home safely from hospital and prevent readmission wherever possible. - Provide support to older people (60+) living in the community to prevent admission into hospital.
The aspects of this service are: (1) Support older people to prepare them for leaving hospital (2) Provides a facilitated discharge to adults of all ages from hospital settings for residents of Bath and North East Somerset (B&NES).
This can include an element of practical housing-related support, cleaning, repairs, or minor adaptations to people's homes to help facilitate their discharge from hospital, or to remain safe once discharged from hospital. (3) Low-level support to older people in the community who are at risk of admission into hospital.
