The brief

There is an identified need to provide high quality domiciliary care in the specialist area of End of Life.

This will support clinical need in the patient's own home, whilst acknowledging the need for sensitive and holistic care.

The service will provide a community based End of Life domiciliary care service for patients with a Stockport GP who require a swift discharge from hospital or increase in care at home due to a rapidly deteriorating condition that may be entering a terminal phase; the service will be required to have a safe package of care ready to commence within 24 hours, 48 hours as a maximum.

This is a service supported by a team of specialist trained carers.

It is anticipated that the service will support approximately up to 200 hours per week of care.

The service supports the timely discharge of CHC fast track patients from hospital as well as avoiding hospitals admissions by supporting people at home with a quick response of care in place in the community.

The service will also greatly improve the safety and experience of patients leaving hospital on a fast track pathway or being supported at home to avoid admission.
