Mid Essex CCG GP Extended Access Service
NHS Mid Essex Clinical Commissioning Group ('the CCG') invite suitably qualified service providers to attend a Market Engagement Event informing the market of the CCG's intentions to commission Extended Access Services in Mid Essex for a minimum of 5 years, and potential option to extend for up to two years.
The Provider must demonstrate they can deliver the 7 core national requirements: - Appointments available Monday to Friday 6.30pm-8pm, plus Saturday, Sunday and Bank Holidays depending on local needs - Capacity - a minimum additional consultation capacity of 30 minutes per 1,000 population - Measurement - use of a nationally commissioned tool to automatically measure appointment utilisation. - Advertising and Ease of Access - included within this, all practice receptionists to be able to direct patients to the service and offer appointments to extended hours service on the same basis as appointments to non-extended hours services and patients to be offered a choice of evening or weekend appointments on an equal footing to core hours appointments - Digital - use of digital approaches to support new models of care in general practice. - Inequalities - addressing health inequalities in the patient population. - Effective access to wider whole-system services - effective connection to other services enabling patients to receive the right care from the right professionals - for example, links to other primary care and general practice services including urgent care.
The Provider; - must sign a Memorandum of Understanding with each of the relevant Practices in Mid Essex to evidence access to their registered list of patients ensuring continuity of provision between Core and Extended periods - must ensure 100% percentage cover from 1st April 2020 for Mid Essex CCG - must deliver the national requirements including 85% utilisation of service - must have access to and utilise a broad spectrum of professionals and skills mix as possible, including but not limited to staff such as GPs, Advance Nurse Specialists, Clinical Pharmacist, physiotherapists, paramedics, Consultant, Long Term Conditions specialist - must collaborate during mobilisation with existing incumbent ensuring seamless transition. - must promote the services to stakeholders, community and secondary care environments, subject to National requirements, working closely with the CCG - must take NHS 111 directly bookable appointments following due diligence process and validation of patient experience. - must demonstrate how they will work with wider integrated healthcare system ensuring Extended Access is managed appropriately - must be able to deliver within the CCG defined locality footprint demonstrating how premises will be secured in advance of service commencement. - must evidence how the proposed IM&T is interoperable with SystmOne, EMIS and Vision and can evidence arrangements with non SystmOne practices. - must have a CQC registration.
