provision for looked after children and young people leaving care from 16 year to 18 year olds and to ensure that in the provision of accommodation the following responsibilities are considered: • To actively promote the health and wellbeing of young people leaving care. • To respect the human rights of young people and their carers, their rights of personal freedom, choice in daily living, dignity and self-respect, independence, privacy, and exercise of free will. • To involve young people and their carers in any decision making which concerns him/her. • To assess and respond to the changing needs of the young people and their carers and to keep significant professionals informed. • To take account of the social, cultural, and religious context of the young people and their carers circumstances when making decisions concerning him/her. • To provide services within the operation of a clear equal opportunities policy. • To provide the requisite care and assistance with regard to any disability, or illness, which a young person may have, and in a manner which complies with the above statements. • To provide good quality local accommodation that allows young people to access education training and employment, maintain contact with family and friends and enable young people to maintain their relationships with significant professionals.
