The brief

NHS Hartlepool and Stockton-on-Tees Clinical Commissioning Group (CCG) proposes to commission a Level 3 Acquired Brain Injury Neuro Rehabilitation Service for the population of Hartlepool and Stockton-on-Tees.

Hartlepool and Stockton-on-Tees Clinical Commissioning Group (CCG) is held to account over their responsibilities to commission provision for people with long-term conditions, including neurological conditions, via the NHS Outcomes Framework.

This includes a number of indicators to measure the outcomes The indicators that can be linked to patients with Neurological Rehabilitation need to include: • Ensuring people feel supported to manage their condition • Improving functional ability in people with long-term conditions • Reducing time spent in hospital by people with long-term conditions • Enhancing quality of life for carers • Improving quality of life for people with multiple long-term conditions The contracting authority will develop the service model to incorporate the Public Services (Social Value) Act 2012, specifically: • How what is proposed to be procured might improve the economic, social and environmental well-being of the relevant area. (Area is the geographic area for which the procurement is taking place) • How, in conducting the process of procurement, it might act with a view to securing the improvement.
