The brief

The Hospital Discharge Rapid Response Service will support and enable the safe and timely discharge of people from hospital who have been assessed as requiring support or provide an assessment of need in the community.

It is intended that the Hospital Discharge Rapid Response service will act as a short-term service where the current or identified provider is unable to support the hospital discharge date with the objective of helping people home from hospital, ensuring safe discharge, getting them established at home and understanding next steps.

Additionally, the service will include 2 x Team leaders who will be available 7 days per week (working alternate weekends) to provide timely assessment in the community on discharge, review support plans, contribute to MDT discussions and facilitate discharges.

The service will work alongside Ethical Home Care agencies, Reablement, other partner agencies and family members to enable a safe and smooth transition where either ongoing commissioned care and support is required, short term reablement intervention or no ongoing support is required.

The service will be available 7 days a week, 365 days a year and will operate between 6am and 11pm.
