The service is tendered under two lots: Lot 1: Occupational Therapy Led Support Enabling individuals to regain independence through physical support and reablement.
Physical support will be aimed at recovering, developing and maintaining the ability to carry out everyday tasks within their home or community.
Lot 2: Cognitive Dementia Professional Specialist support and cognitive input will be aimed at developing an awareness of the reasons behind the behavioral patterns and activities of the individual, which, when understood, will feed into the support tailored for each individual.
The service will deliver a flexible, short-term, promoting independence service.
Service users receive support up to a maximum of six weeks.
This may be on discharge from hospital to enable them to return home safely and to regain their independence, or as a step up from the community to those people who require additional support to maintain their independence.
