Camden CCG has an exciting opportunity for an External Evaluator to assist in assessing a Hospital Discharge Service and Homeless Care Provision.
The Evaluator will start in March/April 2016, a year and a half into the service running to help determine the future model of the contract and inform commissioning intentions.
The Evaluator will report on how successful the service is at improving outcomes for service users and achieving value within Camden’s health, housing and social care system.
The Evaluator will provide critical evidence for commissioners to enable future investments decisions to be made.
