19774 - Tier 3 Specialist Weight Assessment and Management Service The PQQ and ITT stages will be run simultaneously and responses to both must be submitted via Bravo no later than mid day on 22nd May 2015 This specification outlines a Tier 3 services to treat obesity and morbid obesity in a specialised multi-disciplinary (MDT) community setting with an emphasis on psychological support for the increasing numbers of people who have failed to achieve a healthy weight.

Tier 3 would also provide support and preparation for those who go forward to surgical intervention ( bariatric surgery, Tier 4).

Tier 3 weight management services provide medical management for complex obese and severely obese patients: BMI 40+ or BMI 35 + with existing co-morbidities including but not limited to, hypertension, diabetes, CVD and sleep apnoea, high blood pressure, asthma.

The Tier 3 community service will provide specialist assessment and weight management support for adults who have attempted at least a combination of wellbeing services provided by the Public Health department of Local Authority and other commercial weight loss initiatives for at least 6 months.

The service will comprise medical management, dietetic, physical activity and psychological support and contribute to building the capacity of primary care in preventing and reducing obesity and to tlher overall local wellbeing strategy.
