The brief

The Cheshire East Community and Discharge Support Team (CDST) are looking to procure community sector support for Patients leaving Macclesfield and Leighton hospitals as well as people known to Cheshire East's Adult Social Care teams (ASC).

People will require 7 day wrap around support on discharge from hospital or referral from ASC with a view to enabling them to be independent at home, this is likely to include:\r \r o Transport home from hospital\r o Emergency food parcels\r o Hot meal delivery\r o Medication collection and drop off\r o Shopping\r o Wellbeing checks\r o Heating\r o Lighting\r o Initial light cleaning.\r o Signposting to other relevant services for example food banks or befriending for on-going support\r o Obvious home safety issues which require attention\r o Transport to medical appointments\r o Advocacy\r \r The provider will work with Cheshire East Council's Community Support Connectors and its partners, undertaking your own needs assessment to manage the need or refer to your partners as appropriate to achieve the outputs and outcomes required. \r \r The contract is split into 2 LOTS:\r LOT 1 - NORTH\r LOT 2 - SOUTH\r \r
