The Integrated Commissioning Department of the Royal Borough of Kensington and Chelsea (RBKC) have awarded a contract to Dalgarno Neighbourhood Trust in order to deliver a Healthworks Service.
The Healthworks Service shall promote a voluntary sector partnership which will work to improve health in BAME communities predominantly in the North of Kensington and Chelsea.
The Service shall develop an understanding of individual and group service needs amongst BAME communities responding to identified health priorities from within communities.
The Service shall improve service user understanding and ability to prevent and/or manage health needs and work with a cross-sector, multi-agency approach to develop relationships with BAME community groups and voluntary sector organisations.
The Service shall help communities to work together to identify and address the wider determinants of health and wellbeing and develop an understanding amongst service users about what health services are available and how they can be accessed (identifying routes/pathways to health services).
The contract will be up to three (3) years, with the option to extend for a further one (1) year period.
