Market Engagement Event for the Provision of Respiratory Services for North Lincolnshire Clinical Commissioning Group
NHS North Lincolnshire CCG is holding a Market Engagement Event for providers who are interested in tendering for a specialist community respiratory service.
The event will offer an opportunity for potential providers to hear about the CCG's development of a respiratory service in line with the out of hospital model that is being developed as part of the patch wide ‘Healthy Lives, Healthy Futures’ workstream.
It will enable potential future providers to influence the final details of the new service model.
The aims of the integrated respiratory model, and in particular the specialist community provider are: • Triage of all non-urgent GP initiated referrals (except TB, pulmonary fibrosis); • Advice and guidance to general practice; • Accurate and early diagnosis of COPD and adult asthma; • Support appropriate long term management of patients with respiratory disease, including responsible prescribing; • Oxygen prescribing, assessment and reviews; • Integrated pathways of care for patients with acute exacerbations, both in hospital and the community; • Integrated pathways of care to support respiratory patients discharged after an acute admission; • Develop and deliver general practice support to improve the management of COPD and adult asthma; • Work with others to support patients with respiratory disease and mental health problems; • Develop and deliver HCP education and training around respiratory disease in primary care; • Improve self care of people with COPD and adult asthma (including medicines management) through patient education, advice and support to stop smoking; and • Deliver an effective and efficient pulmonary rehabilitation service as part of an integrated respiratory service.
The actual contract length is still under consideration, and the estimated value is approximately £220k per year, dependent upon activity.
The Market Engagement Event will be held on 13 October 2015, at the CCG headquarters in Brigg, North Lincolnshire.
