The Commissioners are looking to secure a neuro rehabilitation service covering bed-based rehabilitation provision including therapy services, as well as community nursing provision for our population (in the region of 267,000).
We are looking for providers who can mobilise a service within a short timescale or around 6 months due to the situation of the current provision.
However due to the national directive regarding the integration of services, it may transpire that the long-term direction is a fully integrated South Yorkshire service.
The primary aim of the service is to provide neuro rehabilitation up to Level 2b, to adults and young people from the age of 16.
The service provides interdisciplinary assessment and treatment to inpatients and in the community.
The model is based on healthcare providers working collaboratively as a single team, with a multidisciplinary approach, to integrate care and deliver a seamless service putting patients and their carers at the heart.
