The brief

The Sheffield Dementia Strategy (2019-2024) was developed together by people living with dementia, their families, and the organisations working to support them.

The strategy aims to make sure that people living with dementia of all ages and their families are supported to live to their full potential.

It includes 13 commitments which describe how we will do this in Sheffield.

Its development was linked to the Prime Minister's 2020 Challenge on Dementia.

The Dementia Short Term Intervention Service was based on feedback from Primary Care and was initially a pilot project established in April 2020 to test a gap in the market.

Its aim is to offer 1-1 support to people living with dementia who are starting to struggle with their situation and present at Primary Care or other contact point (such as Adult Social Care or from the voluntary sector).

The service aims to prevent further deterioration and potential crisis.

The evidence collected during the pilot period has shown the need for, and effectiveness of, this service.

The service will contribute towards improving the care we provide to people with dementia.

It will support people living with dementia who are starting to struggle with their situation and present at Primary Care or other contact point.

It will aim to prevent further deterioration and potential crisis.

The contract is for three years with option to extend for a further two years.

The value given in this notice is for the full five year term.
