NHS North West London Integrated Care Board (NHS NW London ICB) requires the provision of clinically led Community MSK services for the GP registered population aged 16 and over in the North West London borough of Harrow.
The aim of the current service is to provide service users in Harrow with access to Community MSK services, including Community MSK Orthopaedics, Community MSK Rheumatology, Community MSK Physiotherapy (both Direct Access - GP Referrals and MSK Pathway), and Community MSK Spinal Triage.
Key requirements include: • Provision of specialist Multi-disciplinary Team (MDT) MSK care in the community including assessment, diagnosis, treatment and care planning for patients with MSK conditions; • Ensuring that Service Users are appropriately managed in the community; • Improving access to the most appropriate clinician by delivering care closer to home and avoiding unnecessary hospital appointments; • Provision of education and support for GPs; and • Provision of community services that have a strong emphasis on patient education, self-management and pain management, thereby promoting active, healthy lifestyles and reducing recurrence of injury or illness.
The service shall follow any referral guidelines released by the ICB (located at https://www.nwlondonics.nhs.uk/professionals/referral-guidelines-and-clinical-documents). service will be expected to actively manage activity & as a minimum offer Advice & Guidance to GPs & to have an active discharge programme for patients when applicable.
The purpose of this notice is to be transparent about NHS NW London ICB's intention beyond October 2022.
The ICB is committed to developing and delivering common pathways across North West London.
These will be based on optimising patient experience, using best clinical evidence and providing value and improving outcomes.
Over the coming months we will work with system partners to design those pathways and develop how we deliver these models of care in future.
To ensure continuity of service delivery during this process, a new contract will be put into place with the current provider, Harrow Health CIC, from 1st October 2022 for up to 18 months.
The estimated annual value of the contract are £1,930,942.00 (details below).
