The Lewisham NHS@Home Service is a healthcare delivery model designed to provide comprehensive and coordinated care for patients outside of traditional hospital settings.
It leverages technology, remote monitoring, and a multidisciplinary approach to manage the health and well-being of individuals, particularly those with chronic or complex medical conditions.
The objective of NHS@Home is to reduce pressure on the acute healthcare system in Lewisham.

The target population are Lewisham residents, over the age of 18, who would otherwise require hospital level intervention.
