Short term community based service will be targeted at adults who have serious and/or enduring mental health needs to enable them to live and be supported within the community as opposed to acute healthcare settings.
The support offered to Service users will be in line with the clusters 4-17 specifications.
Support may be provided on a one to one or group basis and may include practical help, emotional support, advocacy, links to other agencies and a focus on connecting Service users to universal services there these are needed to sustain their independence.
